About the Role
Raminiscence is DCCT's dementia health and wellbeing programme, delivered in partnership with Primary Healthcare Derby. It runs three distinct offers: one for people who have recently been diagnosed, one for people living with dementia, and one for their carers.
As Dementia Officer, you'll develop and deliver person-centred physical activity, reminiscence, wellbeing and social sessions that help participants stay active, connected and independent. You'll draw on the history and heritage of Derby County Football Club to create meaningful experiences, provide support and signposting, and coordinate referrals with health and community partners.
Building new partnerships and strengthening existing ones is a big part of the job, with the aim of increasing referrals, extending the programme's reach and making sure participants and carers get joined-up support. The role sits within the Trust's Health department and reports to the Adult Health Manager.
What You'll Do
Develop and deliver the three Raminiscence offers for people newly diagnosed, people living with dementia and carers
Offer early support to people who have just been diagnosed, helping them adjust, identify their health and wellbeing priorities, build confidence and find the right activities
Provide ongoing person-centred support for people living with dementia, adapting activities as their needs change
Encourage participants to stay active, connected and involved in their community
Support carers through regular contact, peer connection, health and wellbeing activities, practical information and signposting to local services
Plan and deliver safe, inclusive, dementia-friendly group and one-to-one sessions covering physical activity, reminiscence, social activities and health and wellbeing
Help participants and carers improve their physical and mental wellbeing, reduce social isolation and maintain confidence and independence
Coordinate referrals and participant pathways with Primary Healthcare Derby, Primary Care Networks, social prescribers, dementia services and voluntary and community organisations
Build and maintain strong partnerships that strengthen referral pathways and promote the programme
Promote the programme in local communities that may face barriers to accessing dementia support
Recruit and coordinate volunteers, and organise awareness activity, workshops and community events that support participant and carer wellbeing
Keep accurate records of participants, referrals, attendance and outcomes, and contribute to programme monitoring data
Use feedback from participants and carers to review and improve each of the three offers
What They're Looking For
Essential
A relevant Level 2 qualification in health, social care, community work or physical activity
At least two years' experience supporting people living with dementia, or adults with comparable health and support needs
Experience supporting people newly diagnosed with dementia or another long-term health condition
Experience supporting physical and mental wellbeing through physical activity, social activity or community-based interventions
Experience delivering person-centred group and one-to-one activities
Experience working with health, voluntary or community partners
An understanding of the support needs of people who are newly diagnosed, people living with dementia and carers
An understanding of how physical activity, mental wellbeing, social connection and meaningful activity can support people affected by dementia
The empathy to support people following a diagnosis and help them set achievable health and wellbeing goals
The ability to adapt activities, communication and support as dementia progresses or individual needs change
The ability to recognise and respond to carers' needs, including wellbeing, peer support, information and signposting
The ability to work independently, manage competing priorities and contribute to a multidisciplinary team
Desirable
A dementia awareness or dementia care qualification
A physical activity, exercise, health improvement or Mental Health First Aid qualification
Knowledge of local dementia, health and community referral pathways
